Dental Implants With Gum Disease: Can You Still Get Them?
Dental implants with gum disease are possible when gum health is controlled first. See what research says, how treatment works, and what to expect.

A front tooth implant replaces a visible tooth with a titanium post in the jawbone and a custom crown matched to your smile, so appearance matters as much as strength. At Elia Family Dental in Wall Township, our periodontist places the implant and Dr. Nicole Elia crafts the final crown, and the whole plan stays in one office with dental implants planned from 3D imaging.
A missing front tooth is different from a missing molar. Everyone sees it, you feel it every time you smile or talk, and the replacement has to sit exactly right beside its neighbors. It's reasonable to want to know what the process looks like, how long you'll have a gap, and whether the result will look like a real tooth.
This guide walks through the steps, the timing, the options for a temporary tooth, what affects the look, and who qualifies. It's general education only, and your exam and 3D scan decide what's right for your mouth.
Missing a Front Tooth?
Book a visit and our team will scan your bone and gums and show you what replacing it would involve.
Book Appointment →A front tooth implant is held to a higher standard because it shows. The ADA's guide to dental implants describes them as posts surgically placed in the jaw that function as an anchor for replacement teeth, and says a single-tooth implant is finished with a crown designed to blend in with your other teeth.
In the front of the mouth, three things matter more than usual. The gum line has to follow the curve of its neighbors, the bone supporting the tooth has to be enough to hold that shape, and the crown has to match in color, shape and surface. A small mismatch that nobody would notice at the back is obvious at the front.
Replacing the tooth also protects what's around it. As Elia's implant page notes, over time the bone beneath a missing tooth shrinks and neighboring teeth begin to shift. If you're weighing alternatives, our guide to dental bridge vs implant compares them.
The good news is that planning tools are very good now. A 3D scan, a digital preview and a team that handles both the surgery and the crown make a natural result achievable, as long as the plan is made carefully and in advance.
The steps are a consultation with 3D imaging, digital treatment planning, implant placement by the periodontist, a healing period of about 3 to 6 months, and a final custom crown. Elia Family Dental describes this as a clear, predictable sequence from consultation to your new smile.

The ADA describes the same three phases: the dentist places the implant, the bone heals around it, and then the replacement tooth is attached. Our guides to the dental implant process and CBCT imaging for implant planning explain the first steps in more detail.
Why so many steps? Each one protects the result. The scan prevents surprises, the plan places the implant where the crown will look right, and the healing period lets bone and implant become one unit before the crown adds any load. Skipping a step is how problems start, which is why a careful team doesn't.
Sometimes. A 2023 ITI consensus report concluded that immediate placement with immediate loading in the front of the upper jaw, under favorable conditions, is predictable and associated with high survival rates, although surgical, technical and biological complications can occur. Elia's periodontist offers immediate placement when it's appropriate.
The numbers behind that statement are encouraging. A systematic review of 63 studies found that single implants placed and loaded immediately in the upper front jaw had survival rates of 99.2% at 1 year, 97.5% at 3 years and 95.8% at 5 years, with favorable esthetic outcomes for up to 5 years.
Those results come with a catch. A companion review found that all the studies using this approach reported highly selective inclusion and exclusion criteria, and its authors concluded that survival is high in studies with strict patient and site selection. In plain terms, it works well when the bone, gums and tooth are in good condition, and it isn't right for everyone.
When immediate placement isn't a good fit, there are other routes. Socket preservation after extraction protects the bone while you wait, and our guide to same-day dental implants explains what can and can't be done in one visit. Your 3D scan shows which route suits you.
You don't have to go without a front tooth. The ADA says your dentist may give you a temporary crown, bridge or denture to help you eat and speak normally until the permanent replacement is ready. Your team explains which temporary option fits your case.

The ADA also notes that timing varies. Osseointegration, the process by which bone grows around the implant, takes time, and some patients may need to wait up to several months before replacement teeth can be attached, while other patients can have implants and replacement teeth placed in one visit.
At Elia Family Dental, the healing period runs about 3 to 6 months. During that time, you'll have follow-up visits to confirm the implant is integrating, and you'll keep the area clean as instructed. The last step doesn't begin until the implant is ready to support a crown.
If you're worried about how you'll look during healing, say so at your consultation. It's one of the first questions a good implant team plans around, and the answer is rarely "just live with a gap."
A natural look comes from careful planning of the bone, the gum shape and the crown. Elia's team uses CBCT imaging to plan the implant position, an iTero scan for precise digital impressions with no goopy molds, and a custom crown designed to match your neighboring teeth in color, shape and fit. Digital Smile Design can preview the result.

The crown can attach in two ways, and the choice affects how it looks and how it's maintained. Our guide to screw-retained vs cemented implant crowns compares them, and Elia's implant crown page describes the restoration process. Elia can also restore implants placed by another provider.
Gum shape deserves special attention. The tissue around a front tooth frames it, and its height and contour decide whether the crown looks as if it grew there. That's one reason the implant's position matters so much, and why planning happens before surgery and not after.
Good candidates have healthy gums, enough bone and good general health. The ADA says that if you are in good general health, implants may be an option, and that your health is more of a factor than your age. Chronic illnesses such as diabetes may interfere with healing, and tobacco can slow it.
If gum disease is part of your story, read dental implants with gum disease and diabetes and gum disease for what the research shows. And if bone has been lost, bone grafting before dental implants explains how it can be rebuilt, since Elia's guided bone regeneration develops new bone over about 3 to 9 months. Some people who assume they aren't candidates, because of gum disease or bone loss, find there is a path forward once those issues are addressed. The scan answers most questions in a single visit.
With good care, implants are designed to last for many years. The 2023 systematic review reported 95.8% survival after 5 years for single front implants placed and loaded immediately, and the ADA calls implants an excellent long-term option for restoring your smile. Your long-term result depends on gum health, habits and maintenance visits.
Think of an implant like a natural tooth you must look after, even though it can't get a cavity. The tissue around it can still become inflamed, so daily brushing, cleaning between teeth and regular dental checkups protect it. According to the American Dental Association, regular dental visits can catch 80% of oral health issues before they become serious.
Our guide to dental implant maintenance explains the daily and professional care that keeps an implant healthy. Elia also provides follow-up care after your implant is fully restored, so you're not left on your own once the crown is placed.
One honest note: survival numbers describe groups of patients over a few years, and no one can promise a specific result for one person. What you can do is choose a careful plan, finish any gum treatment first, avoid tobacco and keep your visits.
Cost depends on your bone, whether grafting is needed, the type of implant and the crown, so the clearest answer is a personalized estimate after your exam. Our guides to single tooth implant cost in NJ and what drives dental implant cost explain the factors, without dollar amounts.
Elia Family Dental accepts Delta Dental, Blue Cross Blue Shield, Cigna, Principal and most PPO plans, and verifies your coverage before treatment begins. We'll still work with your plan if you're out of network. Sunbit and CareCredit financing are available, and Sunbit doesn't require a hard credit check. We don't accept HMO, Medicaid or Medicare plans.
If you're considering a front tooth implant, the first step is the scan. Dr. Elia's team will measure your bone, check your gums and tell you honestly what's possible, how long it will take, and what the temporary plan would be. Book a visit, and you'll leave with a clear picture and a next step.
Results may vary. Please consult with your dentist at Elia Family Dental for personalized treatment recommendations.
